usbank home mortgage issue

I visit your office back in march 2014 since then and after us bank mortgage contact me and according to themi have to send lower premim for three month then adter they will sending loan modification application
in march I fill out loan modification application
I call us bank and according to them they receiving all three payment and they told me on june 20 they will sending application and agreement if you agree with agreement then they will reduce mortgage payment
I did not receiving any mail after that other then statement and flood insurance sign up form.
person Craig I try to contact and I like to talk on phone personally and he is not available then I leave message and he not returning my phone call or not leave any message in my cell phone
he is working with loan modification and I do not have any proofor us bank main office doesn't know anbout loan modification !
